1. Health History

During the health history, you and I will meet to review your past and present lifestyle habits, challenges, and concerns. The health history encompasses several different areas including nutrition, mental and medical health, and quality of personal relationships. We will use the health history to develop goals and a plan of action tailored specifically to you and your desired outcomes.

2. Follow Up Session

Follow up sessions will focus on progress toward your goals. This is a time for us to discuss interventions and strategies to move you in the right direction as well as identifying and navigating barriers to your progress. Each session will build on prior ones, and I will provide support, education, and guidance.

3. Final Session

In your final session, we will discuss the status of your goals and how to maintain your new changes and habits. We will also discuss options for continuing our work together if needed and desired. This session is a chance for you to reflect and take stock of all of the shifts you have made on your journey.
